Director of Case Management

Case Management Director

Ideal candidates will have Director-level Case Management experience within a large acute care hospital.

Lead and direct case management services, workflow and resources.

• Formulate and implement operational strategies and initiatives in the case management program.
• Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.
• Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes and issues.
• Partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.

Licensure / Certification / Registration:

• One or more of the following: Case Manager credentialed from the Commission for Case Manager Certification (CCMC) obtained prior to hire date or job transfer date.
• Counselor obtained prior to hire date or job transfer date.
• Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date.
• Social Worker credentialed from the Texas State Board of Social Worker Examiners.
• Respiratory Care Practitioner obtained prior to hire date or job transfer date.
• Respiratory Therapist credentialed from the National Board for Respiratory Care (NBRC) obtained prior to hire date or job transfer date.
• Social Worker obtained prior to hire date or job transfer date.

Education:

• High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required. Master's degree preferred.

Work Experience:

• 5 years of experience required.
• 10 years of experience preferred.
• 2 years of leadership or management experience required.
• 5 years of leadership or management experience preferred.
