What You’ll Do

• Provide care to patients managing chronic conditions
• Educate patients on treatments, medications, and self-care
• Track and adjust care plans for better health outcomes
• Administer medications, document progress, and respond to needs
• Use clinical judgment and autonomy within correctional guidelines
