Job Title : Community Licensed Practical Nurse

STATUS: Full Time

LOCATION: Newark, NY
SCHEDULE: Rotating Days/Evenings with Every Other Weekend Required.
Weekdays: 6am-1pm, 12:30pm-9:00pm, 12:00pm-8:30pm
Weekends rotating shifts 7am-3:30pm or 3pm-11:30pm

SUMMARY
Join our vibrant community care team as a Community Licensed Practical Nurse (LPN). In this rewarding role, you will have the opportunity to provide essential care to ElderONE participants in a variety of settings, helping to maintain their independence and well-being. You will be involved in home visits for important tasks such as dressing changes and medication management and offer support in transitional housing settings. Your contributions are vital in helping participants avoid hospitalization and skilled nursing facility visits, making a significant difference in their lives.

RESPONSIBILITIES

• Preferred experience in home care is desirable, but we are open to working with dedicated candidates willing to gain experience and skills in this area. A passion for working with elderly individuals in diverse environments is essential.
• Collaborate within a team to ensure participants receive the best care possible in their homes or transitional housing.

Minimum Qualifications:

• Graduate from an accredited School of Practical Nursing or an equivalent program required.
• At least one year of experience working with the frail elderly population required.
• Valid NYS Driver's License.

BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

This position requires medium work, involving the exertion of 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Frequent walking, standing, or squatting is required.
