The community you'll be caring for: AdventHealth Manchester

The role you'll contribute:

Under supervision of the Community Health Director, this innovative, grant funded 12/31/2023Community Health Navigator (CHN) position will help assist patients admitted to the hospital with alcohol and/or drug dependence with their insurance and resource needs in preparation for and post discharge. The CHN serves as an advocate for patients, providing support and education so they are better able to engage in their care. This position offers a hybrid work opportunity, with some travel between hospitals for onsite responsibilities coupled with working remotely.

• Educates patients and families about available community resources, programs, and social services.

• Conducts post-discharge phone calls to provide support and encouragement, obtain updates on recovery progress, and collaborate with resource providers for additional assistance.

• Demonstrates good interpersonal communication skills in challenging situations, interacting respectfully, calmly, and courteously with patients, physicians, colleagues, and others.

• Identifies patient needs and helps find resources to address those needs.

• Connects with patients prior to discharge to understand contributing factors and barriers, identifying social and resource needs.

Qualifications

The expertise and experiences you'll need to succeed:

• High School Grad or Equivalent Required

• Experience in a clinical or community health setting (Required)

• Driver's License (DL) Required
