09/24/ th St7th Floor,Denver,Colorado,80202,United States of AmericaThe VP will support key projects in Clinical Transformation and Home in order to help ensure the highest quality of care is given to our chronic kidney disease (CKD) and end stage kidney disease (ESKD) patients. This role is more than a leadership position. It is a chance to help redefine the future of kidney care.
This role will concentrate on managing team members and relationships, building and refining clinical skills among teammates, working with leadership to improve operations, strategy, and innovation, and meeting with team members to ensure that the care and the requisite documentation is of the highest quality.
Clinical Strategy and Operational Oversight
- Overseeing clinical care and care management as it relates to Clinical Transformation and Home in various markets
- Developing and implementing appropriate infrastructure and identifying appropriate
- Participating in model of care development for new programs, e.g. Comprehensive Kidney Care Contracting, communication about the new model and care, and implementation in partnership with physician practices and teammates resources to support DVA initiatives
- Reviewing data related to clinical management and outcomes for patient populations and recommending suggested changes in strategy or process to achieve optimal outcomes
- Participating actively with the Home team regarding strategies to increase Home ESKD treatment penetration in IKC programs
- Implement strategies to increase Home ESKD treatment in IKC programs and review data as to their effectiveness.
- Developing organizational strategy related to clinical teammates and reviewing milestones and metrics pertaining to this and as they relate to DVA initiatives
- Support operations and financial management of DVA initiatives
- Assess operational metrics, productivity, and resource allocation to meet clinical/operational goals
- Lead and support IKC programs as they relate to SNPs, CKCC, and other value-based programs
Clinical Team structure and care delivery
- Working with teams of nurse practitioners (NPs), care coordinators (CCs) and other IKC teammates (TMs) to effectively help them manage populations of ESKD and CKD patients
- Participating in the hiring, managing, and review of clinical team members including NPs, CCs, nurses, and other critical team members
- Meeting consistently with TMs involved in quality of care, other clinical issues, clinical documentation issues, and general business management for DVA initiatives
- Reviewing, amending and updating clinical protocols for care management and care delivery as they
support NPs and other team members in managing their patient populations
- If requested, provide primary care medical services to patients as necessary to support employer programs. All medical services would be provided consistent with state and federal law and licensing requirements and within the VP Physician's scope of practice. VP Physician would be properly insured prior to providing medical services.
Training, Clinical Teammate Development & Compliance
- Participate in individual and aggregated physician practice education about DVA programs, as it relates to Clinical Transformation and Home
- Participate in educational activities to ensure TMs are performing to their utmost capabilities
- Participating in development of training, education, and use of IT systems as they relate to clinical care, clinical documentation and additional aspects of DVA initiatives
Physician Collaboration
- Participate in review of interactions between collaborating physicians and TMs when necessary to ensure compliance with state requirements and quality clinical care delivery
3-5 years Nephrology clinical practice experience; 1-3 years leading clinical organization with NPs; and, 3-5 years prior leadership experience required in roles such as medical director, etc.
Knowledge of clinical practice, population health
Ability to create ideas and deploy new programs in a changing environment, aiding and bringing along new teammates through different stages of growth
Delegated coach and mentor in all aspects. Ensuring the right interactions with teammates, patients, families, physicians, and administrators in order to effectively care for the patient
Manage teammate and patient complaints in a thoughtful and compassionate way, provide high levels of service excellence
Ensure interactions are thoughtful and positive. Have an ability to relate to culturally diverse patients, teammates and communities to teach and train others well
Ability to drive towards metrics and the success that comes with improved clinical and business outcomes
Excellent verbal, written and interpersonal communication skills. The ability to quickly analyze, comprehend and create written/verbal clinical and business communication and documentation
Ability to multi-task in a fast-paced setting. Support teams through different levels of change management.
