Summary:

Join our dedicated team as a Clinical Social Worker specializing in Palliative Care at the University of Colorado Hospital. In this role, you will provide essential psychosocial support to patients in various settings, including inpatient, emergency department, and clinic environments. You will engage in values-based goals of care discussions, advance care planning, and facilitate critical transitions of care, all aimed at improving the quality of life for patients and their families.

Your Key Responsibilities:

• Deliver expert palliative care psychosocial support through direct counseling and collaborative visits, with an emphasis on cultural sensitivity.
• Conduct thorough palliative care psychosocial assessments and document findings comprehensively.
• Work closely with other social workers and the palliative care team to provide exceptional care.
• Lead family meetings to set goals and establish care plans that align with patients' values.
• Help connect patients and families with relevant outpatient services.
• Coordinate educational outreach regarding palliative care resources.
• Utilize strong critical thinking skills to make decisive judgments and work autonomously in a dynamic environment.

Requirements:

• Master's degree in Social Work.
• Current State licensure as a Licensed Clinical Social Worker (LCSW).

Preferred Qualifications:

• Commitment to achieving Palliative Care certification within 2 years.
• At least 2 years of clinical experience in a hospital setting, with a preference for palliative care experience.
