09/18/ Yaupon Dr,Wilmington,North Carolina, ,United States of America Our Team at DaVita is seeking a Patient Educator to join our team with a passion for educating patients about their clinical dialysis treatment options. This person should be passionate about quality patient care as they support and empower patients as they make decisions about how to proceed on their journey with dialysis. This teammate will engage with our leadership team, clinical teams doctors and beyond to ensure that appropriate resources are shared and made available to patients experiencing ESRD. They will serve as the initial point of contact within our teams for patient education and resource sharing. Requirements:

• Strongly preferred to have experience within the dialysis field with a strong understanding of the various dialysis modalities.
• Passion for patient care and patient education
• Ability to work autonomously and as a TEAM in identifying the needs of our patients for additional education and support.
• Ability to travel to multiple facilities within an identified Region.
