Under the supervision of a Registered Nurse Case Manager, the Clinical Resource Management Associate will interact with patients, families, vendors, payers, community agencies, and multidisciplinary healthcare teams to assist in safe transitions of care and appropriate utilization of healthcare services. This role performs various administrative, clerical, and secretarial tasks within the department under little supervision. Provide administrative support to the Director and/or Managers/Supervisors as assigned.

Minimum Education
Associate's Degree (Required)
Bachelor's Degree (Preferred)
Minimum Work Experience
3 years Experience in a Health Care related field. (Required)
Required Skills/Knowledge
Proficient with medical terminology, MS Word, Excel and able to learn the Electronic Medical Record System (EMR).
Strong organizational, interpersonal, and customer service skills required.
Must have ability to prioritize, delegate and manage multiple responsibilities.
Proficient with Microsoft Word and Microsoft Excel skills.
Demonstrates advanced knowledge of office and clerical skills.
Job Functions
- Transitions of Care Collaborate with Case Management, Social Work, and physicians to meet patient/family needs. Utilize Case Management screening tool to gather pertinent information from patient/family. Interact and respond to inquiries from families, payers, external providers, and the medical team by answering questions and explaining information clearly and thoroughly to facilitate the safe transition for patients. Fax patient discharge summaries to identified PCP or provider. Properly documents all relevant actions into the Electronic Medical Record (EMR) systems and other systems as needed.
- Patient Scheduling: Apply knowledge of HSC, its processes, and payer requirements when scheduling appointments. Facilitate scheduling appointments during the patient admission as directed by the medical team. Collaborate with patient's caregivers when scheduling post-discharge appointments (i.e. family preferences for a day of the week, location, etc.).Communicate scheduled appointments to the patient's caregivers and medical team using accepted processes. Collaborate with medical team to facilitate transportation based on patient's identified care needs.
- Performance Improvement Participate in the identification, implementation, and evaluation of processes to improve workflow and customer service. Will support leading or participating in the planning, directing, and implementing of projects as assigned.
- Additional Duties: Supports administrative operations of the Collaborative Services department. Other duties as assigned.

- Light Work: Lifting 25 lbs. maximum with frequent lifting and/or carrying of objects weighing up to 10 lbs.
- This is a full-time, regular position working standard and extended hours.
