Clinical Case Manager Behavioral Health

This position Clinical Case Manager Behavioral Health is with Aetna's Long-Term Services & Supports team and is a field-based position out of West or Central Chicago IL. The requirements is for candidates to hold an Illinois LCSW, LCPC, LMFT, and to travel 50-75% of the time to meet with members face to face.

Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs and improve healthcare program/operational efficiency involving clinical issues. Determines medical necessity/appropriateness. Facilitates optimal outcomes. Identifies and follow through with continuous quality/compliance opportunities. May also include identification of aberrance's and initiation of corrective action. Educates/empowers customers to ensure compliance, satisfaction and promote patient advocacy. Optimize total costs. Implementation and evaluation of policy based on usage and program directives. Educate/empower colleagues at all levels to enable decision making at most appropriate level.

Required Qualifications

-Active IL BH clinical license - LCSW, LCPC, LMFT

-Minimum of 2 years of experience in behavioral health or social services field.

-Must reside in West or Central Chicago and Surrounding Areas, applicable zip codes 60651, 60644, 60624, 60612, 60623, 60622, 60608, 60632, 60616, 60647, or 60639

- Must have a valid IL Drivers License

- Willing and able to travel up to 50-75% of their time to meet with members face to face in West or Central Chicago and Surrounding Areas

- Reliable transportation required; Mileage is reimbursed per our company expense reimbursement policy

Preferred Qualifications

- Case management and discharge planning experience

- Managed Care experience

- Microsoft Office experience

- Experience working with HIV and AIDS population

Education

Master's degree in social services, human services or behavioral health required

Anticipated Weekly Hours 40

Time Type Full time

Pay Range The typical pay range for this role is: $66,575.00 - $142,576.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
