Productivity Rate
- Weekday Rate: $51.33/point
- Weekend Rate: $54.27/point
- Start of Care: 2.2 - 2.5 Points per Visit
- Discharge: 1.0 - 1.5 Points per Visit
- Resumption of Care / Recertification: 1.5 - 1.75 Points per Visit
- Revisit: 1.0 Point per Visit

- Educate patients and their families on proper care techniques, medication administration, and health promotion strategies.
- Communicate effectively with patients, families, and healthcare providers to ensure continuity of care and positive outcomes.
- Participate in ongoing training and professional development opportunities to enhance your skills and knowledge.
- Provide one-on-one skilled nursing care and treatment to patients in their home.
- Assist the registered nurse in assessment of patient's condition and contribute information to aid in planning and evaluating care.
- Demonstrate knowledge of home health and competencies in discipline-specific patient care skills required by the care center to provide patient care.
- Perform comprehensive patient assessments and collaborate with a multidisciplinary care team to develop an individualized plan of care as per physician orders that achieve desired outcomes and goals.
- Check patient's vital signs, Perform wound care, and educate patients and their families about their medications and chronic conditions.
- Provide patient episode management per protocol.
- Complete documentation timely and accurately according to industry standards, conventions, and guidelines, including subsequent visits, or other required documentation.
- Current and unencumbered license as a Licensed Practical Nurse (LPN)
- Strong assessment, critical thinking, and communication skills.
- 1+ year(s) of Certified Home Health experience as an LPN
- Ability to work independently and as part of a dynamic team.
- Comfortability using technical tools and software/EMR for visit documentation
- Current CPR certification.
- Reliable transportation, current driver's license, and car insurance coverage.
- Proof of Immunization/Exemption for Flu and COVID-19.
- TB Test or Chest X-Ray.
