Your Role and Responsibilities

• Collaborate with interdisciplinary teams to design treatment plans tailored to patient needs.
• Establish discharge plans and contingency measures with the care team.
• Oversee the execution of patient discharge processes to ensure a smooth transition.
• Monitor patient experiences and maintain high standards of service quality.
• Facilitate weekly team conferences to discuss treatment plan adjustments.
• Complete necessary documentation and case management reports in a timely manner.
• Stay informed about regulations and company policies to enhance department operations.
• Analyze management reports and take action on improvement indicators.
• Understand payor requirements and participate in meetings to identify trends and solutions.
• Meet with patients and their families to guide them through the rehabilitation process.
• Assist patients with planning resources to avoid discharge delays.
• Ensure accurate discharge documentation and referrals to post-discharge providers.
• Engage in the utilization review process and contribute to quality improvement.
• Participate in on-call case management responsibilities when necessary.

Qualifications

• Licensure/Certification:
• Must have a license that allows for independent assessments (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
• If required in your state, you must hold an active license for your discipline.
• Must be eligible for CCM® or ACM certification upon joining or within two years.
• CCM® or ACM certification is required or must be obtained within two years of starting.

• Minimum Qualifications:
• Nursing candidates should have at least an Associate Degree in Nursing, with RN licensure (BSN preferred).
• All other licensed or certified health care professionals must possess at least a bachelor's degree (advanced degrees preferred).
• A minimum of 2 years of rehabilitation experience is preferred.
