Summary

The primary emphasis of the RN Case Manager is to partner with all other members of the health care team to assure efficient care of inpatients, avoiding delays and denials whenever possible. The Case manager, working with health care team, strives to achieve the best outcomes of appropriate utilization and safe, efficient and timely transition and discharge planning functions. Requirements include an associate's degree, 2-4 years related experience and/or training, CPR - AHA Healthcare Provider, and RN - Registered Nurse.
