Become the Case Manager You Aspire to Be

• Collaborate with an interdisciplinary team to guide effective treatment plans based on patient needs.
• Coordinate with the team to establish and refine discharge plans.
• Engage in planning and enhancing the patient discharge experience.
• Monitor and assess patient experience for quality and compliance.
• Lead weekly team conferences and coordinate treatment plan modifications.
• Complete necessary documentation accurately and on time.
• Stay informed about industry regulations and organizational standards.
• Analyze case management reports and take action as needed.
• Attend ACT meetings to discuss trends and solutions.
• Meet with patients and families promptly after admission to guide their journey.
• Assess goals and complete documentation within required timelines.
• Educate patients and families about rehabilitation processes and case management roles.
• Facilitate family conferences to ensure everyone's involvement.
• Assist patients in procuring necessary resources to prevent discharge delays.
• Ensure compliance with regulations regarding orthotics and prosthetics.
• Make timely referrals and ensure accuracy of patient records.
• Participate in the data collection and review process for utilization management.
• Be available for case management on-call duties as necessary.

Qualifications

• Must be qualified to independently assess patients within your discipline (e.g., RN, SW, OT, PT, ST, or Rehabilitation Counseling).
• If licensure is required, you must hold an active license for your discipline in your state.
• Must meet eligibility for CCM® or ACM certification upon beginning this position or within two years.
• CCM® or ACM certification is required or must be obtained within two years of starting the Case Manager II position.

Minimum Qualifications:

• For nursing positions, an Associate Degree in Nursing is required; RN licensure with BSN preferred.
• For other licensed or certified healthcare professionals, a minimum of a Bachelor's Degree is required, with a graduate degree preferred.
• A minimum of two years of experience in rehabilitation is preferred.
