We are looking for a compassionate professional to join our team and leverage your social work skills to empower individuals and families. As a proactive problem solver dedicated to client well-being, you will play a crucial role in advocating for patients and providing crisis intervention.

Job Summary

In this role, you will provide:

• Supportive services and counseling related to healthcare and home care programs.
• Assistance with the social, emotional, and economic concerns of patients, their families, and caregivers.
• Coordination and planning of programs to help individuals achieve or maintain optimal levels of functioning.
• Crisis intervention and assistance in understanding medical situations.
• Collaboration among patients, families, and healthcare teams to deliver comprehensive care.

You'll also respond to suspected abuse, neglect, or violence, adhering to ethical guidelines and state laws. Develop tailored care plans by sourcing necessary resources from health and human services. Participate in care conferences and transitional rounds while demonstrating professionalism and commitment to our organization.

Qualifications

Social Worker:

• Bachelor's degree in Social Work from an accredited program.
• In Minnesota: Licensed Social Worker (LSW).

RN Care Manager, Inpatient:

• Graduation from a nationally accredited nursing program.
• Bachelor's Degree in nursing or completion of the degree within 18 months of hire.
• Current unencumbered RN license with the State Board of Nursing.
