Become the Case Manager You Aspire to Be

  • Collaborate with an interdisciplinary team to tailor treatment plans that meet patient needs.
  • Coordinate the development of tentative discharge plans and contingency measures with your team.
  • Participate in planning and executing patient discharge experiences.
  • Monitor patient experiences across various quality indicators, service appropriateness, and compliance with expectations.
  • Facilitate weekly team conferences to adjust treatment plans as necessary.
  • Complete required documentation and case management addendums.
  • Stay informed about regulations, standards, and company policies that shape department operations.
  • Review and analyze case management reports, including Key Care Indicators, to shape appropriate actions.
  • Understand various commercial contract levels and payor requirements.
  • Attend Acute Care Transfer (ACT) meetings, identifying trends and solutions for improvement.
  • Meet with patients and families within 24 hours of admission, educating them on the rehabilitation process.
  • Provide timely assessments and case management addendums within 48 hours of admission.
  • Establish communication plans to ensure clarity and support for patients and families.
  • Assist with resource planning to prevent discharge delays.
  • Ensure compliance in ordering and payment for orthotics and prosthetics.
  • Facilitate referrals for post-discharge services to ensure continuity of care.
  • Verify accuracy of discharge information and related payor details in patient records.
  • Engage in the utilization review process, including data collection and trend analysis.
  • Participate in the case management on-call schedule as necessary.

Qualifications

  • Licensure/Certification:
    • Must be qualified to independently complete assessments according to your discipline (RN, SW, OT, PT, ST, or Rehabilitation Counseling).
    • If licensure is required in your state, you must hold an active license.
    • Must qualify for CCM® or ACM certification upon entering this position or within two years.
    • CCM® or ACM certification is required or must be obtained within two years for Case Manager II roles.

Minimum Qualifications:

    • If qualifying as a nurse, you must have at least an Associate Degree in Nursing; RN licensure is required, with BSN preferred. A diploma is acceptable only in states where it is the minimum requirement for licensure.
    • For all other eligible licensed or certified health care professionals, a minimum of a bachelor’s degree is required, with a graduate degree preferred.
    • At least two years of rehabilitation experience is preferred.
