Nurse Manager - Case Management Team (Continuum of Care)

Key Responsibilities:

• Lead the Case Management team by implementing best practice models and supporting continuous improvement initiatives.
• Collaborate with physicians and healthcare providers to ensure safe and effective patient care.
• Manage daily operations, oversee compliance, and address escalated issues to best support our team and patients.
• Monitor resource utilization and implement care delivery systems that enhance the continuum of care.
• Foster professional development through training and constructive feedback to support employee growth and performance.

Minimum Qualifications:

• Bachelor's degree in Nursing (BSN) with at least three years of clinical healthcare experience.
• A minimum of two years in a leadership role is required.
• Registered Nurse License (Washington) or Compact License upon hire.
• Basic Life Support certification is required at hire.
