Care Manager RN

Description

We are seeking a dedicated and experienced Registered Nurse (RN) Care Manager to join our Population Health team. This role is ideal for a nurse with a strong background in chronic condition management who is passionate about building and shaping a new care management program. The RN Care Manager will work collaboratively with Social Work Care Managers, Community Health Workers, and a Quality Team to deliver patient-centered care and improve health outcomes.

Key Responsibilities:

• Lead and support the development and implementation of a Care Management program
• Provide comprehensive care management to patients with chronic conditions, focusing on high-risk and rising-risk populations.
• Support the Transitions of Care (TOC) team as needed
• Conduct assessments, develop care plans, and monitor patient progress toward health goals.
• Engage patients and families in self-management education and support.
• Participate in interdisciplinary team meetings and contribute to workflow improvements.
• Ensure compliance with billing and documentation standards for CCM and TCM services.
• Track and report on patient outcomes, productivity, and program impact.

Required Qualifications:

• Washington Registered Nurse License upon hire

Preferred Qualifications:

• Bachelor's Degree in Nursing
• Certified Case Manager upon hire
• 2 years of Clinical experience
• In utilization review/case management
