Responsibilities:
- Completes an assessment of client and family to determine home care needs, making necessary revisions to care plan per client/family requests or as needs change
- Demonstrates the ability to think critically in exercising professional judgment
- Consults the family, staff and/or external care providers when the client has changes in condition
- Ensures our clients receive "best in class" non-medical care management
- Always maintains a calm, positive demeanor and can effectively multitask in a high functioning office environment
- Trains caregivers to maintain a safe and healthy working environment and uses safe work procedure
- Promotes the growth of Right at Home by representing the organization, its mission, and its role in the community in a professional manner
- Builds personal rapport and establishes trust with clients, family members, and caregivers
- Effectively communicates and interacts with clients, caregivers, and families in a supportive manner
- Provides education to clients and family members on resources, self-care techniques, and/or medical conditions that the client may need to seek from a medical physician, Home Health Company or Hospice agency
- Plays a key role in clients avoiding unnecessary visits to the ER and/or hospitalizations
- Addresses concerns or complaints voiced by clients, families, or caregivers, follows up with the leadership team with recommendations and next steps
- Acts as a role model for new caregivers, accompanying them on home visits and/or acting as a mentor
- Maintains productivity according to Right at Home standards
- Develops training plan(s) and trains caregivers in the skills required for non-medical home care
- Fosters the professional development of self and other care staff
- Prepares, records, and maintains care plan and progress tasks/notes in the client care record in an accurate, timely manner and per Right at Home policy and procedure
- Meets required deadlines for documentation care plan updates, addressing progress towards goals
Requirements:
- Graduate of an accredited practical or vocational nursing or CNA program and licensed to practice
- 5+ years of nursing experience (home health/hospice preferred)
- Must possess and maintain a current CPR certification
- Must have a valid driver's license and use of an insured automobile for work or access to adequate transportation
- Available for work on weekends as required
- Participate in on-call rotation for office staff
- Must be able to read, write, speak and understand English
- An ability to bend, squat, raise your arms and lift up to 50 lbs without restrictions
Compensation: $50,000.00 - $55,000.00 per year
