Care Coordinator/Liaison Naples, FL

Visiting Angels is actively seeking an enthusiastic Care Coordinator/Liaison to cultivate and strengthen relationships with community stakeholders in Naples and surrounding areas . This role is central to promoting our services, driving care consultations, and building strategic partnerships for our non-medical home care organization.

The ideal candidate will be a dynamic relationship-builder with local connections, passionate about making an impact in the lives of seniors and their families.

What Youll Do

• Conduct care consultations with potential clients to assess needs and develop personalized care plans.
• Build and maintain partnerships with referral sources through educational initiatives, presentations, and events.
• Drive sales and marketing efforts to expand business, leveraging both new and existing community relationships.
• Provide support to current clients through quality assurance visits and case management when needed.
• Engage with caregivers, addressing their needs and reinforcing their importance as valued team members.

What Were Looking For

• Proven experience in building and nurturing positive relationships to support business growth.
• Case management experience (highly preferred).
• Home Care or Healthcare background (strongly preferred).
• Ability and willingness to travel throughout Naples and surrounding areas to meet clients, families, and referral partners.

Note: Our Administration Office is located in Bonita Springs, FL, with a Satellite Office in Naples, FL. This position will focus on the Naples area.
