Additional Information About the Role

• PT Shift Information: Monday through Friday 8:00 Am - 4:30 PM - scheduling is based on PT hours - 24 hour per week

• Home Office Location: - Mason Ridge in Chesterfield

• Coverage area: St Charles County

• Case Load: 17 - 20

• Job Responsibilities: - Meeting with patients/families, determine resource needs and making referrals, determine social support needs, education and planning for end-of-life needs, counseling support needs for anticipatory grief.

• Preferred skills or required work experience: home health experience is not required, but a bonus. Additionally, this is a very independent position, prior social work experience is preferrable

Overview

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

The Metro St. Louis Intermittent Home Care Department of BJC Home Care Services provides home visits to patients in the metropolitan St. Louis area and several nearby counties, with 24 hour on-call home care nursing supervision. Role Purpose

Under direction of a physician, assists patients and their families in their residences to adapt and plan an interdisciplinary approach to treating the patient-family system.

Responsibilities

• Consults with patients, families and staff regarding goals of care, end of life issues and palliative needs.

• Provides direct clinical services to patients and families.

• Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.

• Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
