Bilingual Chinese RN Care Manager - Remote with 1 Field Day a Week!

Join our dedicated team as a Nurse Care Manager, where you will make a significant impact on the lives of our members through effective care coordination. Your role will involve conducting in-home assessments, creating individualized care plans, facilitating services, and advocating for our members to ensure their health needs are met with quality and cost-effective solutions.

Key Responsibilities:

• Work closely with members and their families to help maintain their independence in the community.
• Monitor and assess members' needs regularly to ensure continuity of care across the healthcare continuum.
• Authorize necessary services and collaborate effectively between various health payers.
• Communicate proactively with members, families, caregivers, healthcare providers, and interdisciplinary teams to coordinate optimal care.

Qualifications:

• Current NYS RN License and registration.
• Associate's Degree in Nursing is required; Bachelor of Science in Nursing (BSN) is preferred.
• At least 3 years of RN experience required.
• Preferred clinical expertise in geriatrics; experience in managed long-term care is a must.
• Bilingual proficiency in Chinese (Cantonese or Mandarin) is required.

Compensation and Benefits:

• Annual salary: $105,000 - $115,000.
• Eligible for a $20,000 sign-on bonus ($5,000 paid every six months, after taxes).
