The BHRC Counselor provides direct support to patients; develops and implements individualized treatment plans, observes and documents client response to interventions, and provides therapeutic support for patients and their families (mediation, locating resources, and providing mental health education). Works with a professional team of behavioral health staff to provide excellent care and customer service to patients served.

Registered interns in process of receiving licensure to be considered in lieu of licensure. BLS, provided by American Heart Association (AHA), required within 30 days of employment. Completion of NAPPI training (Non-Abusive Psychological & Physical Intervention) within 90-days of employment; annual re-certification required thereafter. Level 2 DCF Background Check required upon hire.

Responsibilities
- Prepares diagnostic formulations for use in development of the individualized treatment plan and discharge planning, and Level of Care (LOC) considerations. Confers with members of the professional clinical staff throughout all patient treatment processes.
- Demonstrate professional knowledge and skill with various therapy modalities including SBIRT, and crisis intervention.
- Confers with patients (and family/other supports) to identify treatment needs and develop individualized treatment and discharge plans.
- Demonstrates the knowledge and skills necessary to provide quality outpatient addiction recovery/behavioral healthcare, based on the physical, psychosocial, educational, and cultural and safety criteria appropriate for each patient's developmental stage.
- Participates in the Interdisciplinary Treatment Team (IDTT) in order to coordinate treatment plans, discharge planning, LOC determination, community referrals when needed.
- Provides individual, and family therapy for patients in order to achieve identified short term therapeutic goals and assists with warm hand off to community partner agencies for ongoing services.
- Documents patient assessments, treatment progress notes, social service and discharge planning interventions in the medical record.
- Works closely with Care Connect + staff to assess for and address any social determinants of health as well as assessing for any additional barriers to continued treatment.
- Maintains a referral source database, including outpatient Psychiatrists and Psychotherapists, other mental health/substance abuse providers, housing, vocational and recreational resources, etc. in order to provide the least restrictive, appropriate referral.
- Demonstrates an advanced level of psychosocial, crisis intervention and addiction/recovery assessments and services and knowledge and expertise in the use of the Diagnostic Statistical Manual (DSM) 5, established standards of professional and ethical practice, Florida Mental Health statutes, and hospital and departmental policies and procedures.
- Maintains all records and documentation according to hospital policy and procedure.
- Performs all other duties as assigned by management within job scope.
Education / Training
- Master's Degree in Social Work, Mental Health/Mental Health Counseling or Marriage & Family Therapy
- 2-years Mental Health and Substance Abuse/Addiction Recovery
- Basic Life Support (BLS)
- NAPPI (WELLE) Certification
- Level 2 DCF Background Check
