Department: BH Integration Shared Services

FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks)

Shift: Days

Pay: $31.61 - $47.41 / hour. Pay is dependent on applicant's relevant experience

Responsibilities:

• Provide high-quality counseling and therapy to assigned patients through individual and group sessions.
• Conduct comprehensive interviews and assessments of patients as well as their families and caregivers to inform treatment plans.
• Coordinate and arrange necessary internal and external services to support patient needs, participating in case reviews to monitor progress.
• Assist in discharge planning, identifying community resources, and providing referrals to ensure a smooth transition.
• Utilize effective teaching methods to meet the educational needs of patients and their families.

Minimum Requirements:

• Master's Degree.
• State licensure as a Licensed Professional Counselor Candidate (LPC-C), Marriage and Family Therapist Candidate (MFT-C), Social Work Candidate (SWC), or Licensed Social Worker (LSW).
