Behavioral Case Manager

Banner Thunderbird Medical Center is looking for a passionate Behavioral Case Manager to join their team. The Behavioral Health Unit at Banner Thunderbird Medical Center is made up of 56 beds and cares for a wide range of different mental health diagnoses as well as those behavioral health patients who have co-morbidities that require an extra level of acute medical care.

As a Behavioral Case Manager, you will have the opportunity to work with a wide variety of diagnostic categories and age groups. Our case managers are scheduled to work 5 consecutive days and we provide 7 days a week coverage. The schedule will be Monday-Friday 8:30a-4:30pm with some weekend on-call rotation. The psychiatrists, nurses, and Behavioral Health case managers work closely as a team to strategize the treatment approach. There will be ample opportunity to practice your family therapy skills with pediatrics, and group therapy skills and addiction counseling skills with our adults. This position provides comprehensive care coordination for patients as assigned. This position assesses the patient's plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient's health care needs. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for the quality of clinical services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care.

Core functions include managing individual patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes, providing assessment, direct psychoeducation, and direct psychotherapy to individuals, families, and groups, and acting in a leadership function with process improvement activities for populations of patients to achieve the optimal clinical, financial, operational, and satisfaction outcomes.

Minimum qualifications include a master's degree in social work or related field or equivalent experience, proficiency level typically achieved with 5 years clinical experience, and a licensure in at least one of the following categories: social work, professional counseling or marriage and family. Preferred qualifications include additional related education and/or experience.
