Primary Responsibilities

• Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues
• Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population certified in the Practice while administering quality patient care
• Maintains accurate and confidential medical records; documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations
• Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications

Qualifications

• Unrestricted licensure in the state of New York at time of employment
• Licensing: Registered Nurse with current unrestricted license to practice and use the title of Nurse Practitioner/Advanced Practice Nurse in NYS AND/OR Licensed PA in NYS
• Unrestricted DEA license (or willingness to obtain prior to start)

Additional information

The salary range for this role is $110,000 to $166,500 annually based on full-time employment. Salary range is defined as total cash compensation at target; actual base and bonus mix may vary by experience, metrics, and location. UnitedHealth Group complies with all applicable wage laws.
