As an Infectious Diseases Clinical Pharmacist Specialist, you will work alongside our ID teams to provide direct patient care for patients on ID consult services. While on clinical service, as the ID Clinical Pharmacist Specialist you will be responsible for the management and monitoring of antimicrobials for patients on the ID consult service and provides clinical expertise to enhance patient outcomes. You will promote antimicrobial stewardship through monitoring and reviewing use of antimicrobials within the hospital. Additional antimicrobial stewardship responsibilities include the conducting review of all positive blood cultures with real-time intervention and feedback, implement AS initiatives to promote optimal antimicrobial use, participate in clinical research and quality improvement projects in collaboration with pharmacy, infectious diseases, clinical microbiology, nursing and other healthcare services, assist in developing and maintaining antimicrobial formulary, guidelines and use evaluations, as well as orders sets as directed through system-wide ID groups. As the ID Clinical Pharmacist Specialist, you will also provide ID pharmacy services through the continuum of care by assisting the OPAT/CoPAT service by conducting laboratory monitoring and optimizing antimicrobial selection. You will also serve as a mentor to clinical pharmacists and residents regarding clinical judgement and clinical practices and will play a role in the PGY2 ID Pharmacy Residency Program.
