Conducts comprehensive health assessments focused on wound, ostomy, and continence care. Performs targeted physical evaluations and develops individualized care plans based on patient diagnoses and needs. Recommends appropriate medications and treatments while ensuring high-quality, evidence-based care.

Key Responsibilities:

• Develops, revises, and evaluates standards of care; assists in selecting appropriate supplies, technology, and equipment.
• Assesses and documents patient responses to wound, ostomy, continence, fistula, and complex wound treatments.
• Evaluates the quality and effectiveness of care provided by clinical staff within the unit.
• Collaborates with the healthcare team and initiates referrals to other disciplines when necessary.
• Educates and counsels patients and families on prevention and management of complications related to wounds, ostomies, and continence issues.
• Develops, delivers, and evaluates staff education programs, including new employee orientation, in-services, and continuing education sessions.
• Conducts comprehensive assessments of patients at risk for pressure injuries and implements preventive strategies.
• Communicates and escalates challenges, including ethical concerns and conflict resolution matters, to appropriate leadership.
• Works closely with the Assistant Director of Nursing for Wound Care to recommend and implement best practices.
