Qualifications:

Licenses & Certifications:
- CA RN License required
- BLS required
Requirements:
- Must have current work history within 2 years.
- 2-year acute Hospital Case Manager experience performing Care Coordination
- Discharge Planning in a very fast paced environment
- Experience with HMOs, IPAs, etc.
- Competent with arranging DME, SNF and ambulance transport.
- Must have acute care experience!
- Experience with Epic
- Ratio - 1:20
