Job Description

American Traveler is hiring an experienced RN Case Manager with strong acute care and assessment skills for a day shift Case Management position.

Responsibilities

• Work in a hospital-based Case Management department
• Day shift, Monday through Friday with 8-hour shifts
• Part of a multidisciplinary care management team coordinating care for selected patient populations
• Position does not accept local candidates within a 50-mile radius
• Coordinate patient care, discharge planning, and resource procurement for patients and families
• Monitor patients’ progress and address changes in medical needs affecting current and future treatment
• Collaborate closely with the healthcare team, patients, families, and external payers
• Assess patient goals, discharge needs, and financial status as part of comprehensive care planning
• Holiday coverage may be required as per unit needs

Requirements

• Active RN license in Oklahoma or a Compact (eNLC) RN license
• Graduate of an accredited nursing program
• Current BLS certification from the American Heart Association
• Minimum of one year of clinical nursing experience required; three years of acute care experience strongly preferred
• Strong clinical assessment skills and knowledge of acute care nursing practices
• Knowledge of third-party payer issues and reimbursement regulations
