Job Description

American Traveler is hiring an RN Case Manager with at least 3 years of RN experience and 2 years in case management for a day shift position.

Responsibilities

• Work in a hospital case management unit
• Coordinate interdisciplinary care from admission to discharge
• Day shifts from 08:00 to 16:30
• 13-week contract assignment
• Responsible for creating and implementing care plans across the continuum
• Monitor patient medical necessity and coordinate care transitions
• Act as a liaison and advocate for post-discharge planning and resource removal
• Maintain clear communication with care team and ensure accurate documentation
• Local candidates are accepted at a reduced pay rate, as determined by facility’s radius rule
• Covid vaccination is no longer required
• Auto-offer process for all placements

Requirements

• Active IN or compact RN license required
• Minimum 3 years of RN experience
• At least 2 years of case management experience
• Current BLS certification from AHA
• Nursys verification and primary source license verification within 30 days prior to start
• US Social Security Number and US home address required
• Submission of 2 professional references and copy of Driver’s License required
