The Good Life MedStaff is seeking a travel Physical Therapist for a travel job in Santa Clara, California.

Locations: need PTs in the following locations:
• Central Valley (Stockton/Manteca/Modesto)
• Oakland/Berkeley
• Martinez
• Hayward/Fremont
• Santa Clara/San Jose
Mileage: We can bill for mileage at standard IRS rate
Setting: OP - Home Health
Experience: 1+ year of recent Home Health experience,
• knows Kinnser/Oasis and can commit to doing minimum of 25 visits per week.
Certs: BLS/CPR should be AHA accredited/certified
Start Date: ASAP
How Many Weeks: 13 weeks
Shift: 5 x 8

Guaranteed hours/week: 40 hours if 25 visits per week requirement is met, see additional details below
License: CA PT
Certifications: BLS/CPR
Setting: Home Health
Mileage Reimbursement Rate: standard IRS rate
Years of Experience Required: minimum of 1 year in Home Health setting
Additional Experience / Requirements Preferred: Acute care experience is a plus
Traveler will perform a minimum of 25 visits per week to attain guaranteed 40 hours. No guaranteed hours the first week for orientation and training. Guaranteed hours reduced by 8 hours (5 visits) for each holiday day. Client guarantees to provide/schedule a minimum of 25 visits per week for the Traveler (able to accept minimum 5 SOCs per week, if offered). If the Client fails to provide the minimum 25 visits, the Traveler will be paid the guaranteed 40 hours. If Traveler fails to complete 25 visits per week, when offered, Client shall deduct 1.6 hours at the regular bill rate per visit under 25 visits. Client shall pay the Traveler 1.6 hours at the regular rate for each visit performed above 25 visits with no overtime paid. As we discussed, we pay 1.6 hours for each visit completed. We do not pay overtime, just 1.6 hours per visit. If we fail to give the Traveler 25 or more visits per week, we will pay the guaranteed 40 hours. If Traveler fails to complete 25 visits when offered, then the guaranteed 40 hours is nullified.
