Current CPR certification
Valid driver's license, auto insurance, and reliable transportation

Home Health Nursing Responsibilities

Provide skilled nursing care to home health patients according to individualized care plans
Conduct comprehensive patient assessments and document findings using OASIS and HCHB
Collaborate with interdisciplinary healthcare teams to improve patient outcomes
Educate patients and families on disease management, medications, and self-care
Monitor patient progress, update care plans, and communicate changes to the healthcare team
Ensure accurate and timely clinical documentation for all nursing care provided
