We’re seeking a compassionate and motivated Travel or Local Contract Registered Nurse (RN) to join our team in a top-tier Home Health company in Baltimore, MD. Whether you're looking to hit the road or explore a temp-to-perm opportunity, this role offers both professional growth and adventure.

Assignment Highlights:

• Take-home pay: $2000/$2200 week (after taxes)

• Health, Dental, Vision benefits – start Day 1

• Schedule: Mon-Fri (8-hour days) no weekends

• Mileage Reimbursement: Patient to patient home

• 40-Guaranteed Hours (Based on productivity points)

Requirements:

• Valid RN license

• Current CPR certification

• Home Health experience required, Including Oasis SOC

Your Role:

• Identify and link patients with community resources to facilitate referrals and respond to social service needs
• Track and support patients when they obtain services outside the practice like Emergency Departments, Hospitals and other health care facilities
• Follow-up with patients within a few days of an emergency room visit or hospital discharge
• Under the direction of the practitioners, communicate test results and care plans to patients/families
• Provide relevant self-management support for patients with chronic illnesses as identified by clinical team.
