Patient Population: Dependent on unit – Adult Medical/Surgical, Cardiac, Ortho and possibly Peds● Skills required:o EPIC experience is requiredo Skilled in placement for SNF/IPR/LTAC/Hospice/Home Health, DME, Behavioral Health needs/DSS (peds), Difficult complex placement, will work routinely with homeless populationo Care management skills: assessment, care coordination and discharge planning
