Essential Responsibilities: Conduct comprehensive patient assessments to determine medical, psychosocial, and discharge needs.Develop personalized care plans in collaboration with physicians, nurses, and therapists.Monitor patient progress and treatment effectiveness to adjust plans as needed.Maintain accurate records of patient interactions, care plans, and interventions.Prepare reports for hospital administration and insurance providers to ensure compliance.Participate in quality improvement initiatives to enhance patient care efficiency.Requirements:New Mexico/Multi State RN License- requiredCurrent Basic Life Support issued by the American Heart Association- requiredCovid Vacc Card- required
