Position Overview

Seeking a compassionate LPN to join a Primary Care / Community Health team providing comprehensive patient-centered care in an outpatient setting. The ideal candidate will assist in routine assessments, medication administration, and care coordination while ensuring a positive patient experience.

Experience & Requirements

Experience: Prior Primary Care, Outpatient Clinic, or Community Health experience preferred
Certifications:
- BLS (Required)
- Active LPN License (North Carolina or Compact)
Travel Experience: Not required
EMR Experience: Epic or comparable system preferred

Core Responsibilities

Support providers in patient care, including vital signs, medication administration, and specimen collection
Assist with health assessments, wound care, and chronic disease monitoring
Maintain accurate and timely documentation in EMR
Promote patient education and preventive health practices
