POSITION OVERVIEW:

As a Home Health Licensed Practical Nurse (LPN), you will deliver compassionate and skilled nursing care to patients in their homes. Your primary focus will be on promoting health, preventing disease, and assisting patients in managing chronic conditions. You will play a crucial role in coordinating care with physicians, therapists, and other healthcare professionals to ensure the highest standards of care and patient safety are met.

KEY REQUIREMENTS:

• A minimum of 1 year of recent experience in home health care.
• Current Illinois State Healthcare Provider license or willingness to obtain.

Certifications Needed:

• BLS certification may be required.
