Job Description

American Traveler seeks an experienced RN for a Home Health & Hospice position serving a diverse patient population, requiring home health and hospice experience and on-call rotation.

Responsibilities

• Provide nursing care in a home health and hospice setting across a 60-mile radius
• Care for adult, geriatric, and pediatric patients in private homes and nursing facilities
• Day shifts M-F 8a-5p with on-call rotation for evenings, weekends, and holidays (24-hour coverage required during weekends/holidays)
• Utilizes HomeCare HomeBase EMR system
• No self-scheduling; participation in on-call and holiday rotation required
• Solid color scrubs may be worn
• Conduct initial and ongoing patient assessments, develop care plans, and implement nursing interventions
• Administer medications and treatments as prescribed
• Provide patient and family education based on assessment and plan of care
• Act as case manager and coordinate care as assigned
• Document clinical notes and update physicians as needed
• Coordinate necessary equipment and services for home care
• Supervise and evaluate home health aides
• Holiday and on-call coverage is mandatory
• No additional RTO will be approved after initial agreement
• First-time travelers are considered

Requirements

• Active ND or compact RN license required at time of consideration; pending licenses not accepted
• Minimum 1 year of RN experience in home health and hospice required
• Current BLS certification required
• Valid driver's license and auto insurance required
• HomeCare HomeBase EMR experience preferred
• Must not have held full time, part time, or PRN employment with any CommonSpirit-associated facility within the past year
• Permanent residence must be at least 50 miles from the facility
