Salary Range: $30.00 - $51.00
Pay Info: Social Worker: $30.00 - $36.00, RN Care Manager, Inpatient: $34.00 - $51.00

Key Responsibilities:

• Provide supportive services and counseling related to healthcare and home care programs.
• Engage as a vital member of an interdisciplinary team to address social, emotional, and economic concerns of patients and their families.
• Offer crisis intervention and assist families in understanding complex medical situations.
• Coordinate healthcare programs among patients, families, and healthcare teams.
• Respond to suspected abuse, neglect, or violence in line with ethical policies and state laws.
• Develop tailored care plans by collaborating with social and health service agencies.
• Provide resources, referrals, and education on available programs and services.
• Demonstrate commitment to organizational goals by participating in special projects and care conferences.
• Exhibit critical thinking and decision-making skills in patient care.
• Build rapport with patients, families, and community healthcare teams to ensure effective communication.

Qualifications:

For Social Worker:

• Bachelor's degree in Social Work from an accredited curriculum required.
• Licensed Social Worker (LSW) in Minnesota.

For RN Care Manager, Inpatient:

• Graduate from a nationally accredited nursing program required.
• Bachelor's Degree in Nursing required or completion within 18 months of hire.
• Must hold an unencumbered RN license with the State Board of Nursing.
