Job Description:
Under the supervision of the Director of Nursing, the Registered Nurse (RN) provides direct patient care, supervises Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs), and collaborates with other healthcare professionals to ensure optimal patient outcomes in a skilled nursing facility and rehabilitation center setting.
Responsibilities:

Patient Care:
- Conducts comprehensive nursing assessments and develops individualized care plans.
- Administers medications and treatments as ordered by physicians.
- Monitors patient conditions and initiates appropriate interventions.
- Provides wound care, ostomy care, and other specialized treatments as needed.
- Educates patients and families about disease processes, medications, and self-care.
- Collaborates with the rehabilitation team to develop and implement patient rehabilitation plans.
Leadership and Supervision:
- Supervises LPNs and CNAs in providing patient care.
- Delegates tasks appropriately and ensures staff compliance with facility policies and procedures.
- Participates in the hiring, training, and evaluation of nursing staff.
Quality Improvement:
- Participates in quality improvement initiatives to enhance patient care outcomes.
- Identifies and reports potential or actual incidents and adverse events.
- Collects and analyzes data to identify trends and opportunities for improvement.
Documentation:
- Accurately and timely documents patient assessments, interventions, and outcomes in the electronic health record.
- Completes required documentation, including incident reports, medication administration records, and progress notes.
Collaboration:
- Collaborates with physicians, therapists, social workers, and other healthcare professionals to provide coordinated patient care.
- Participates in interdisciplinary care planning meetings.
Compliance:
- Ensures compliance with state and federal regulations, including OSHA, HIPAA, and CMS guidelines.
- Maintains current licensure and required certifications.

Qualifications:

Graduate of an accredited nursing program.
Current and valid Registered Nurse (RN) licensure in the state of Ohio.
Experience in long-term care, skilled nursing, and rehabilitation preferred.
Strong clinical assessment and problem-solving skills.
Excellent communication and interpersonal skills.
Ability to work collaboratively as part of a healthcare team.
Demonstrated leadership and supervisory abilities.
Proficient in electronic health record documentation.
