RN - Hospital at Home $10/K BONUS
Schedule: 4 Days weekly, 12p-10p, 3 weekend shifts per month

Responsibilities:
- Performs patient assessment and provides care.
- Educates patient on their condition, prescribed medications, nursing
- Interventions and care plans.
- Participates in the coordination of services with other disciplines and community resources as needed.
- Identifies and acts on changes in the patient's condition.
- Evaluates and revises care plans/assessments based on changes in patients and/or their environments.
- Documents all assessments and interventions.
- Collects specimens as required (including phlebotomy).
- Administers medications including IV medications as ordered.
- Utilizes and instructs patient/family on the use of the telehealth tablet used for biometric data.
- Participates in regular case conferences and in-service or special case orientations as needed and participates in on-call rotation.
- Performs other duties as assigned.
- Asociate science in nursing required, BSN preferred.
- Two years of RN experience, three to five years of inpatient RN experience in med/surg preferred.
- BLS.
- Currently licensed to practice in the state in which you are required to work.
- Experience providing RN services in the home, community or clinic is preferred.
