Responsibilities

• Coordinates the provision of hospice services for each patient and family
• Direct and follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
• Proactive, collaborative team member in the hospice environment

Licensure, Certification, and Educational Requirements

Current, active, and unrestricted Registered Nurse (RN) licensure in state of practice
