Overview

The Resource Navigator works as part of an intensive residential treatment team that provides care, treatment and rehabilitation for persons served who are experiencing significant mental health/psychiatric symptoms; who are demonstrating significant functional impairments; and who may have co-occurring disorders.

Responsibilities

• Crisis Assessment and medical/behavioral emergency.
• Assists persons served in obtaining all financial, medical and community benefits.
• Maintains current directory of community resources.
• Assists in discharge planning and care coordination.
• Assists Clinical Director in obtaining prior authorizations.
• Collects rent from persons served or rep payee.
• Manages client invoices for Room and Board.
• Maintains an updated waitlist of referrals.
• Writes Progress notes and summaries.
• Provides Group and Individual Counseling services.
• Skills Teaching.
• Case Management.
• Household management.
• Implements treatment/rehabilitative activities.
• Crisis Prevention.
• Medication Administration.
• Participates in weekly staff meetings.
• Reports significant incidents or accidents.
• Establishes mutual trust and support.
• Provides coaching/learning environment.
• Rewards good performance and promotes recognition.
• Promotes consumer choice and independence.
• Promotes community integration and choice making.
• Notifies authorities of health hazards.
• Observes and implements policies.

Qualifications

• At least a bachelor's degree in behavioral science.
• Minimum of 2000 hours of work experience.
• Or Master's degree in behavioral sciences.
• Case Management experience preferred.
• Experience in managing systems, processes, and people.
• Ability to communicate verbally and in writing.
• Operate general office equipment.
• Good organizational skills and attention to detail.
• Capable of working with confidential information.
• Must be able to work independently and as part of a team.
• Dependable, accepts responsibility for attendance and punctuality.
• Conduct self in professional and ethical manner.
• Cooperates and is willing to assist others.
• Demonstrates productive individual and teamwork behaviors.
• Communicates in an open and honest manner.
• Moderate travel (25-75%)

Salary Range

USD $53,000.00 - $55,000.00 / Year
