Direct Support Program Coordinator

In the Direct Support Program Coordinator role, you will help this company in its commitment to serving others by providing direct care to clients/individuals receiving services and coordinating the development and implementation of services/treatments provided to clients/individuals.

  • Assist in accessing medical and health-related services for individuals receiving services; provide on-call coverage for caseload and participate in rotational emergency coverage and crisis intervention support as needed.
  • Assess referred clients and participate in the development of a service plan in conjunction with a treatment team and funding source following corporate, contractual, and regulatory standards. Coordinate and evaluate the plan implementation.
  • May assist manager/director with direction and supervision of assigned staff, including input on recruitment, performance evaluation, and discipline.
  • Implement and actively contribute to the development, review, and revision of programs and behavior intervention plans.
  • Schedule and/or provide opportunities for quality training, recreation, and leisure activities with consideration for choice, preference, and age-appropriateness.
  • Provide monitoring, consultation, and comprehensive support services to Mentors/staff regarding work with individuals supported and implementation of service plans on an ongoing basis. Make home visits and conduct meetings as required.
  • Maintain current and complete client and Mentor/staff reporting records following Mentor and regulatory standards. Assist with all legal, licensing, regulatory, and certification activities as needed.
  • Provide pre-service and in-service training to mentors/staff as assigned and assist in mentor training, planning, and development.
  • May secure individual Mentor contracts; complete annual contract reviews for Mentors; and provide timely information as to any changes in client status and status of Mentor homes to management.
  • Maintain current records for all independent contractors on caseload including training, monthly and annual mentor contract compliance forms, respite and support plans, and tracking of respite support work.

Qualifications:

  • High School diploma or equivalent
  • Three years related work experience
  • Current driver's license, car registration, and auto insurance if providing transportation to individuals receiving services.
  • All state-required training and certification completed in mandated timeframes.
  • Heavy work. Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.
  • Effective communication skills (both verbal and written)
  • Ability to meet due dates (formal and self-imposed) with minimal supervision
  • Ability to independently solve practical problems
  • Acute attention to detail and high quality in all deliverables
