Job Summary

The Licensed Practical Nurse (LPN) is responsible for delivering professional nursing care to residents across all age groups in a long-term care environment, under the supervision of a registered nurse, advanced practice provider, or physician. You will utilize your nursing knowledge and skills to meet the physical and psychosocial needs of residents, as outlined in their care plans.

You will:

• Ensure the health, comfort, and safety of residents.
• Administer medications and treatments as ordered.
• Keep accurate documentation of resident interactions and care outcomes.
• Communicate effectively with residents, family members, and healthcare providers.
• Participate in interdisciplinary care teams to achieve the best outcomes for residents.

Qualifications

• Graduation from an accredited practical nursing program is required; an associate degree is preferred.
• If applicable, completion of an IV Therapy course approved by the state board of nursing is necessary.
• Current unencumbered LPN license in the state of practice, with multi-state licensure privileges if applicable.
• Willingness to complete required annual continuing education units and maintain department-specific competencies.
