Job Title: Practical Nurse - ElderONE

SUMMARY:
As a Practical Nurse, you will play a vital role in delivering compassionate and dedicated care to our patients under the guidance of a Registered Nurse. Your skills and commitment will directly impact the well-being of our elderly population and enhance their quality of life.

RESPONSIBILITIES:

• Thoroughly observe and document each patient's medical status upon admission and throughout their treatment journey.
• Support the implementation of individualized treatment plans as directed by the Registered Nurse.
• Deliver routine nursing services and care to patients as needed.
• Adhere to evaluation, treatment, and documentation protocols established by the department.
• Ensure timely and accurate completion of required documentation.
• Provide regular updates on patient status to the appropriate team members.
• Participate actively in departmental and facility meetings as necessary.
• Maintain compliance with all hospital regulations, professional standards, and policies.
• Enhance your clinical skills by pursuing ongoing education opportunities.
• Perform additional duties as assigned to support patient care.

REQUIRED QUALIFICATIONS:

• Graduated from an accredited School of Practical Nursing or equivalent.
• At least one year of experience working with the elderly or frail population.
• Current licensure as a Practical Nurse in the State of New York.

PREFERRED QUALIFICATIONS:

• Strong communication, customer service, and interpersonal skills.
• Experience working with a frail or elderly population preferred.

EDUCATION:

LICENSES / CERTIFICATIONS:

BLS - Basic Life Support - American Heart Association, LPN - Licensed Practical Nurse - New York State Education Department.

PHYSICAL REQUIREMENTS:
Medium Work - Must be able to exert 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds frequently, and/or greater than negligible up to 10 pounds constantly. Frequent walking, standing, or squatting is required.

PAY RANGE: $27.75 - $33.75

The base pay range listed above represents a good faith estimate of current potential base pay for a successful applicant and is subject to modification. Pay is influenced by various factors including experience, relevant qualifications, specialty, internal equity, location, and contractual agreements.
