This is a full time, 14-week hybrid summer internship that will occur during one of the below time frames:
- May 4 August 7, 2026

- May 18 August 21, 2026

Job Summary
The DS Population Health Management team provides essential support to Care Delivery in the domains of Care Management, Care Coordination, Data Analytics/Reporting, and patient engagement, offering a comprehensive range of services for both inpatient and outpatient settings.
Our primary focus lies in the Epic applications: Healthy Planet, Compass Rose, and Cheers-Campaigns. We collaborate closely with our operations team to develop initiatives aimed at keeping patients healthy and reducing hospital admissions across our populations. This is achieved through preventative care programs, health maintenance reminders, targeted health campaigns, and continuous optimization of Epic's suite of tools to enhance workflows, minimize administrative burden on caregivers, and streamline patient care.
As a SAFe Agile Team, we intend for our intern to work alongside each of our three sub-teams, ensuring a well-rounded and immersive experience. This would include having mentors in the role of Lead Application Analyst and Lead Scrum Coach, as well as building relationships with the full team. The internship may also provide opportunities for SAFe and Epic training.
Essential Functions
- Maintains accountability for completing support tasks as directed.
- Maintains clear and appropriate documentation of actions taken.
- May analyzes department processes and other Corewell Health systems encountered and makes recommendations improvements, as appropriate. Participates in quality monitoring activities/process improvement activities.
- Communicates, collaborates, and acts as a team player with others in order to ensure continuity and coordination of services.
Qualifications
Required
- High School Diploma or equivalent
- Must be Sophomore or Junior standing and is not expecting to graduate before May 2027.
