Summary: DHMF Dominican Internal Medicine is looking for a Locums physician, details below!
First to Fill
Rate Caps for Submission:
Malpractice - Must be included in rates.
Orientation - half the hourly/callback rate*** kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"
Hourly - *** and below - "Per hour, not OT"
Holiday - Must be 1.5x the regular rates "Per holiday hour, no OT"
Schedule/Availability Requirements:
ASAP- as soon as credentialed - 6 months to a year
•Shift times: Weekdays: 8:00 am to 5:00 pm, with a 1 hour lunch
•Frequency: Monday - Friday, every week- no call, would also consider someone who could do only a few days/week
Facility Location: 1595 Soquel, Suite 400, Santa Cruz, CA
Setting: Internal Medicine- will consider a strong FP but that is not ideal.
FTE: 1.0
# Providers Needed: 1
Duties: Typical IM clinic population average age 70. About half male/female with multiple comorbidities, such as diabetes, arthritis, heart disease, chf, etc
Required Procedures: N/A
EMR: Cerner
Patients per Day: Other providers seeing 12-18 a day depending upon complexity (includes new patients)
Support Staff: MA's, MOR's, APP (one in clinic) and RN assigned to clinic
Reason for Coverage: Provider Retiring
Deal Breakers? Not board certified
Travel Preferences:
Preferred Lodging: Our Locums stay in hotels or in AB&B up to the CommonSpirit cap
Air Travel: Would prefer someone local, but wouldn't rule out someone who wants to fly home on the weekends.
Credentialing Information:
Timeframe: 2-3 months

Credentialing with other facilities? DHMF ONLY
Billing enrollment: Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? No
SUBMISSION REQUIREMENTS
Board Certification - BC in Internal Medicine required, Geriatrics is a plus!! Site will consider candidate BC in Family Medicine if they have experience in IM & Family Medicine.
State License - California required
Residency Requirements/Fellowship - N/A
Other Certifications - BLS
Malpractice - Clean, strongly preferred
