• Board Certification: Board Certified or Board Eligible within 3 years of Residency in Pediatrics - REQUIRED
• State License: Active OR License - REQUIRED
• Background Check: Clean Malpractice/Background - REQUIRED
• Availability: Must be available at the time of name clear (must note if available with the specified dates) - REQUIRED
• DEA: Required during Credentialing
