We are seeking a compassionate and skilled Physical Therapist Assistant to join our dynamic home health team. In this role, you will collaborate with physical therapists to empower patients in their journey to regain mobility and alleviate pain, all from the comfort of their own homes.
Key Responsibilities:
- Assist physical therapists in delivering therapeutic treatments and procedures tailored to each patient’s care plan.
- Monitor and document patient progress throughout sessions, communicating any changes or concerns to the supervising physical therapist.
- Support patients in performing prescribed exercises and utilizing therapeutic equipment safely within their home environment.
- Educate patients and their families on exercises and activities to maintain progress between visits.
- Adjust interventions within the treatment scope while adhering to state regulations.
- Ensure timely and accurate documentation to meet agency and regulatory standards.
Work Environment:
- This position is rooted in providing home health care, necessitating travel within the community and adaptability to diverse home settings.
- You will utilize strong interpersonal skills and the ability to work autonomously to support patient goals effectively.
